I am using Thief Gold + TFix (not TFix Lite) + AngelLoader and both locks spawned for me. I don't know what happened there. Maybe try the full TFix package. You don't need to install the optional mods if you don't want to (just untick them while using the installer).

The starting point isn't the only exit. There is actually another exit underground after you find the Chalice, but I won't spoil any more.

I was playing on Expert and had to rely on the YouTube walkthrough attached to this page to find an extremely hidden (read "invisible") key, and there was also a well-hidden switch. Spoiler below if you don't wanna skim through the video:
